Healthy Pregnancy, Healthy Baby Program

This program has been designed to assist you with useful information, resources and tips to help educate and guide you through the completion of your pregnancy.
This program will also provide you with critical answers, for both planned and unplanned pregnancies.
Other perks of the program include:
- A Healthy Pregnancy and Healthy Babies Educational Booklet
- A Welcome Gift
- The opportunity to win cash incentives upon completion of requirements.
Attached you will find the Enrollment Instructions, Enrollment Form/Informed Agreement, and FAQ for the program. Please thoroughly review the paperwork and return all completed forms to the Risk and Benefits department (Attention: Wellness) or email to wellness@osceolaschools.net
PROGRAM INSTRUCTIONS
We commend you on your decision to participate in the Healthy Pregnancy, Healthy Babies Program. Please follow the instructions below to complete the enrollment process.
- Read the enclosed Healthy Pregnancy, Healthy Babies Program Frequently Asked Questions.
- Complete the Enrollment Form and sign the Informed Agreement/Consent. Email completed forms to wellness@osceolaschools.net or send to Risk and Benefits (attn: wellness)
- Complete the To-Do-List throughout your pregnancy AND keep it for your records until the last week of pregnancy. Also, you must submit proof of Orlando Health Virtual class completion. Both copies must be submitted upon completion of the program to be eligible for the cash incentive.
Once we receive the Enrollment Form and the Informed Agreement/Consent you will receive a welcome letter, a welcome gift and an educational Healthy Pregnancy Healthy Babies booklet for you to review and use throughout your pregnancy. Please do not hesitate to contact us if you have any questions about the program or need assistance completing the enrollment forms. Your enrollment package can be mailed, faxed or scanned using the following information:
